What is happening
An anal fissure is a tear in the lining of the anus, usually from hard stool or straining, causing sharp pain on passing stool that lingers afterwards, with bright red blood. Piles and fistula-in-ano are similarly common and similarly treatable. What matters is not assuming - bleeding from the back passage should always be examined, because the same symptom can come from something more serious.
What helps
- Fibre of 25-30 g a day and plenty of fluid
- Sitz baths for 10-15 minutes, three or four times a day
- Treat constipation properly rather than straining
See a doctor promptly if you have
- Bleeding that continues despite treatment
- Pain with fever or a lump
- Any change in bowel habit alongside the bleeding
How we investigate it
Almost always the answer comes from a careful history first, and only then a targeted test — not a panel of everything. Depending on what we find, that may be an endoscopy, a colonoscopy, a Fibroscan or a breath test.
